Mistakes often occur because care team members lack clarity about&lt;br /&gt;exactly what they are to do, or when they are to do it. Research has shown that&lt;br /&gt;asking a question early (rather than asking the same question later) not only&lt;br /&gt;reduces the risk of failure or injury, but also saves time and reduces the stress&lt;br /&gt;that comes with ambiguity and uncertainty.&lt;br /&gt;An example: An intern joined rounds late. At the end of rounds, the senior house&lt;br /&gt;officer noted that it was time to remove the first of the two chest tubes in an&lt;br /&gt;elderly patient. While everyone present nodded in agreement, the task was given&lt;br /&gt;to the late intern, who did not know which tube was placed first and did not feel&lt;br /&gt;comfortable asking after having nodded knowingly moments before. He&lt;br /&gt;incorrectly assumed the patient was the one that he participated in rounds on.&lt;br /&gt;Later, the intern went to carry out the task, and incorrectly removed what looked&lt;br /&gt;like the older tube. Had he asked questions when first assigned the task, he&lt;br /&gt;would not have removed the wrong chest tube from the wrong patient. This&lt;br /&gt;caused the patient respiratory distress requiring placement of another chest tube.&lt;br /&gt;Reference: Sexton, J.B. (2004). DURING TIMES OF HEAVY WORKLOAD,&lt;br /&gt;LEADERS SHOULD MANAGE THE SITUATION WHILE OTHERS&lt;br /&gt;MANAGE THE ACTUAL TASKS.&lt;br /&gt;Why? Research has shown that such a strategy reduces multitasking and&lt;br /&gt;improves decision-making and vigilance. Leaders who fail to delegate during high&lt;br /&gt;workload risk increased errors and accidents due to the burden of trying to&lt;br /&gt;simultaneously managing the tasks, the team, and the environment. Evidence of&lt;br /&gt;this phenomenon comes from a handy statistic in commercial aviation. The&lt;br /&gt;National Transportation Safety Board found a disproportionately high percentage&lt;br /&gt;of aviation accidents (over 80%) occur when the captain is the pilot doing the&lt;br /&gt;actual flying and the first officer is the pilot "not flying." These captains are&lt;br /&gt;overloaded as they try to accomplish flight tasks and command duties at the&lt;br /&gt;same time.&lt;br /&gt;An example: in our own industry involved a charge nurse failing to delegate. A&lt;br /&gt;patient with a history of an elevated heart rate spontaneously went into a fast&lt;br /&gt;irregular rhythm. The bedside nurse called out for help when the rhythm caused&lt;br /&gt;a significant drop in blood pressure. The charge nurse, three additional bedside&lt;br /&gt;nurses, and the medical resident arrived at the bedside. The resident ordered an&lt;br /&gt;intravenous medication, which the charge nurse went to obtain and then&lt;br /&gt;administered. Rather than delegate the task to the other staff members, which&lt;br /&gt;would allow her to better supervise and document the efforts of the staff, the&lt;br /&gt;charge nurse sent the other three nurses away stating she could handle the&lt;br /&gt;situation. The patient did not respond to the medication and the physician&lt;br /&gt;ordered that the patient be cardioverted (an electrical shock to trigger a normal&lt;br /&gt;rhythm) and that a continuous medication be given intravenously. The charge&lt;br /&gt;nurse left to mix the medication and get the defibrillator machine. The charge&lt;br /&gt;nurse returned to administer the electric shock with the physician present. The&lt;br /&gt;patient continued to deteriorate and needed additional treatment that was&lt;br /&gt;delayed because the charge nurse did not delegate tasks to the other team&lt;br /&gt;members. Memberikan penjelasan kepada kontraktor terkait dengan aspek Keselamatan dan Kesehatan Kerja dan Lingkungan ( kebijakan K3L, prosedur, peraturan – peraturan dll ) di PT XXX INDONESIA.&lt;br /&gt;b. Menjamin keselamatan kerja para karyawan dan kontraktor pada saat melakukan pekerjaan di area perusahaan.&lt;br /&gt;&lt;br /&gt;2. KEBIJAKAN K3L PT XXX INDONESIA. &lt;br /&gt;1. Melakukan perbaikan secara terus menerus terhadap pengelolaan Lingkungan, Keselamatan &amp; Kesehatan Kerja (K3L) untuk mencapai kecelakaan nihil dan menghilangkan potensi bahaya.&lt;br /&gt;2. TATA CARA BEKERJA AMAN &lt;br /&gt;a. Sebelum bekerja di area perusahaan, kontraktor harus mengisi form ijin kerja kontraktor. ( disetujui oleh Manager seksi, dan SHE COM )&lt;br /&gt;b. Pimpinan kontraktor harus memastikan bahwa peralatan K3L yang dipersyaratkan dalam bekerja telah dipenuhi dan digunakan oleh karyawannya.( Alat pelindung diri, Alat pemadam kebakaran,  dll )&lt;br /&gt;c. Kontraktor harus mematuhi peraturan yang berlaku di PT XXX INDONESIA .&lt;br /&gt;d. SHE COM / Security sewaktu-waktu akan memeriksa bahwa persyaratan K3L telah dipenuhi.&lt;br /&gt;e. Apabila kontraktor melakukan pekerjaan yang termasuk dalam pekerjaan khusus maka kontraktor harus mengikuti ketentuan yang ada dalam prosedur ijin kerja khusus.&lt;br /&gt;f. Apabila di dalam pekerjaan tersebut mengandung potensi bahaya untuk terjadinya pencemaran lingkungan maka kontraktor harus mengikuti ketentuan yang ada dalam prosedur ijin kerja khusus.&lt;br /&gt;g. Apabila  di dalam pekerjaannya kontraktor menggunakan bahan kimia berbahaya dan beracun maka perlu melakukan komukasi tentang cara penanganan dan pembuangannya.&lt;br /&gt;h. Setelah selesai bekerja kontraktor harus membersihkan tempat kerja.&lt;br /&gt;4. APD ( ALAT PELINDUNG DIRI )&lt;br /&gt;NO APD FUNGSI&lt;br /&gt;1 Helmet Melindungi kepala dari benda keras, saat terjatuh, kejatuhan benda.&lt;br /&gt;2 Topi Sebagai identitas untuk membedakan antara karyawan, kontraktor, dan lain-lain.&lt;br /&gt;3 Sabuk Pengaman Mencegah jatuh dari atas pada saat bekerja di temapat yang tinggi.&lt;br /&gt;4 Kaca mata safety Melindungi mata dari benda kecil / serbuk yang mengenai mata.&lt;br /&gt;5 Kaca mata las Melindungi mata dari percikan api dan sinar las.&lt;br /&gt;6 Kedok las Melindungi muka dan mata dari percikan api dan sinar las.&lt;br /&gt;7 Masker kain Melindungi alat pernapasan dari masuknya debu.&lt;br /&gt;8 Masker Gas Melindungi alat pernapasan dari masuknya zat berbahaya ( gas, bahan kimia dan bau  yang menyengat )&lt;br /&gt;9 Sarung tangan katun Untuk menangani benda yang panas dan tajam &lt;br /&gt;10 Sarung tangan kulit Untuk menangani benda yang lebih panas sehingga tidak bisa menggunakan sarung tangan katun.&lt;br /&gt;11 Sepatu safety Melindungi kaki dari kejatuhan benda berat, menginjak benda tajam.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;5. KETENTUAN PENGGUNAAN API DI DALAM PERUSAHAAN&lt;br /&gt;a. Pada dasarnya dilarang menggunakan api didalam perusahaan, kecuali yang berkaitan langsung dengan proses produksi dan pembuatan / perbaikan alat yang rutin. Untuk mencegah terjadinya kebakaran peusahaan, menyediakan alat – alat pemadam  seperti Alat Pemadam Api Ringan ( Dry chemical, CO2 ), Hidran, pasir dll ditempat yang berpotensi terjadi kerbakaran.&lt;br /&gt;b. Tempat-tempat yang diperbolehkan menggunakan api : &lt;br /&gt;- Radiator soldering&lt;br /&gt;- A/C (Car, bus) Aluminium Brazing&lt;br /&gt;- Parts ( Press Welding )&lt;br /&gt;- Pengelasan di machinery dan carpenter   &lt;br /&gt;c. Pekerjaan yang sama sekali tidak boleh dilakukan di lingkungan perusahaan. &lt;br /&gt;- Membakar dokumen, seharusnya dokumen di potong-potong kemudian dibawa keluar untuk dibakar. Pada saat membakar diawasi oleh PIC dokumen.&lt;br /&gt;- Pembersihan hunger radiator, A/C dengan cara dibakar, seharusnya dibersihkan dengan palu, bila dibersihkan dengan cara dibakar harus dibawa keluar ( subcont )&lt;br /&gt;- Membakar sampah&lt;br /&gt;- Mengelas disekitar painting booth, seharusnya jauh dari painting booth ( lebih dari 10 M )&lt;br /&gt;- Memotong/mengelas cerobong painting booth&lt;br /&gt;- Mengelas di dalam gudang.&lt;br /&gt;- Mengelas di dekat LPG Storage&lt;br /&gt;d. Pekerjaan yang boleh dilakukan, tapi harus mendapatkan ijin dari safety committee&lt;br /&gt;- Pekerjaan pengelasan yang tidak rutin, seperti memperbaiki bangunan, alat dsb. Wellhead dan X-Mas Tree&lt;br /&gt;Wellhead / X-Mas Tree (Kepala Sumur) adalah suatu peralatan yang berfungsi untuk:&lt;br /&gt;• Memisahkan/menahan rangkaian casing &amp; tubing. &lt;br /&gt;• Memisahkan tekanan annulus yang satu dengan yang lain.&lt;br /&gt;• Memisahkan tekanan annulus dengan tubing.&lt;br /&gt;• Tempat duduk X-mas tree, yang terdiri dari rangkaian beberapa valve seperti SV 1(Safety Valve 1), SV2, MV (Master Valve) dan Top Valve.&lt;br /&gt;Mengatur/mengontrol aliran pada waktu aliran fluida produksi dan tekanan dari dalam reservoir.&lt;br /&gt;&lt;br /&gt;Biasanya di kepala sumur ini dipasang alat-alat keselamatan untuk menjamin dalam pengoperasiannya, seperti alat-alat monitor tekanan (pressure gauge), ini perlu dikenal sebelum membuka ataupun menutup valve-valve tersebut (lihat gambar-gambar wellhead &amp; X-Mas Tree).&lt;br /&gt;Seorang operator produksi biasanya bekerja di daerah kepala sumur senantiasa memperhatikan  prosedur-prosedur kerja,  sebelum dan sesudah menutup salah satu valvenya. Yang perlu diperhatikan didalam bekerja di lapangan minyak adalah bekerja dengan  Tekanan dan Temperatur Tinggi.&lt;br /&gt;Sehingga perlu diperhatikan setiap aktifitas yang berkaitan dengan alat-alat produksi dan dapat membahayakan keselamatan pekerja itu sendiri. Perhatikan instruksi ataupun prosedur kerja yang telah dibuat oleh pegawai yang berwenang agar senantiasa mematuhinya, apabila ingin melakukan diluar instruksi ataupun prosedur mintalah ijin atau petunjuk dari pegawai yang berwenang. Jangan sekali-sekali membuka &amp; menutup valve di kepala sumur tanpa prosedur kerja ataupun ijin dari atasan yang berwenang.&lt;br /&gt;&lt;br /&gt;2. Test Separator (Bejana Pemisah Fluida)&lt;br /&gt;Adalah suatu suatu bejana yang berfungsi untuk memisahkan gas, fluida minyak, kondesat dan air.&lt;br /&gt;&lt;br /&gt;Ada 2 macam separator adalah sebagai berikut :&lt;br /&gt;• 2 phase separator.&lt;br /&gt;2 phase separator berguna untuk memisahkan semua cairan dari gas.&lt;br /&gt;• 3 phase separator.&lt;br /&gt;3 phase separator berguna untuk memisahkan gas, air dan minyak.&lt;br /&gt;&lt;br /&gt;Ada 3 jenis separator adalah sebagai berikut (lihat gambar separator):&lt;br /&gt;• Separator Horizontal.&lt;br /&gt; Biasanya digunakan untuk memisahkan minyak dengan gas yang mempunyai GOR tinggi (Gas Oil Ratio/Perbandingan Gas dan Minyak).&lt;br /&gt; Digunakan untuk kapasitas pemisahan yang besar.&lt;br /&gt; Mudah pemasangannya.&lt;br /&gt; Baik untuk menerima aliran yang stabil.&lt;br /&gt;• Separator Vertikal.&lt;br /&gt; Biasanya digunakan untuk pemisahan minyak dengan GOR yang rendah.&lt;br /&gt; Baik untuk menerima aliran yang relatif tidak stabil.&lt;br /&gt; Mudah dibersihkan.&lt;br /&gt; Pemasangannya tidak memerlukan tempat yang luas&lt;br /&gt;• Separator Spherical ( Bulat).&lt;br /&gt; Intermediate dari kedua diatas &lt;br /&gt; Lebih padu dari tipe yang lain.&lt;br /&gt; Kapasitas surge cairan dan ruang pemisah yang terbatas.&lt;br /&gt;&lt;br /&gt;Dengan mengenal peralatan-peralatan untuk menangani proses hydrokarbon di sebuah proses industri perminyakan sangat besar pengaruhnya seperti : &lt;br /&gt;• Perbedaan atau persamaannya dalam pegunaan alat alat tersebut mengenal fungsi dari Main separator, test separator dan auxillary separator dsb&lt;br /&gt;• Perbedaan nama-nama dari separator adalah sangat memegang peranan penting pada saat bekerja dengan aman/safe di proses fasilitas produksi hydrokarbon baik untuk lapangan gas atau minyak.&lt;br /&gt;&lt;br /&gt;Bekerja di bejana yang mempunyai tekanan &amp; temperatur tinggi, harus tetap mematuhi aspek keselamatan didalam pengoperasiannya. Tekanan separator biasanya tinggi dan dilengkapi oleh suatu valve yang dikenal Pressure Safety Valve (PSV)  yang fungsinya melepaskan tekanan yang berlebihan yang telah diset apabila terjadi di separator tersebut.&lt;br /&gt;&lt;br /&gt;Telah diterangkan di atas bahwa mengenal dan mengetahui secara pasti fungsi dan kegunaannya dari peralatan produksi adalah hal penting dalam pencegahan kecelakaan, apabila seseorang diberi tugas untuk memonitor atau mengoperasikan peralatan separator, yang perlu diperhatikan adalah&lt;br /&gt;• Tekanan di dalam alat tersebut,&lt;br /&gt;• Maximum tekanan,&lt;br /&gt;• Temperatur kerja&lt;br /&gt;yang diijinkan sesuai standard desain peralatan tersebut.&lt;br /&gt;&lt;br /&gt;3. Pipeline&lt;br /&gt;Pipeline adalah suatu alat transportasi/alat penyalur untuk mengirim fluida atau gas dari satu tempat ketempat lainnya.&lt;br /&gt;• Pipa lepas-pantai (Offshore pipelines) :&lt;br /&gt;Berfungsi untuk mentransfer kandungan hydrokarbon (Gas alam, Minyak bumi, kondesat dan air) dari satu proses anjungan minyak ke anjungan minyak lainnya atau sebaliknya ke tempat penyimpanan baik di lepas pantai ataupun didarat.&lt;br /&gt;• Pipa penyalur di darat (Onshore pipelines) :&lt;br /&gt;Sama fungsinya adalah penyalurkan kandungan hydrokarbon (Gas alam, Minyak bumi, kondesat dan air) dari satu lapangan minyak ketempat penyimpanan atau dari satu daerah kedaerah lainnya.&lt;br /&gt;Pipa-pipa ini diklasifikasi berdasarkan dari arah mana aliran fluida mengalir di fasilitas produksi tersebut, sehingga dapat dibedakan sbb: &lt;br /&gt;1. Incoming line (aliran masuk).&lt;br /&gt;2. Outgoing line (aliran keluar).&lt;br /&gt;3. Bidirectional (dapat digunakan untuk aliran masuk atau keluar).&lt;br /&gt;&lt;br /&gt;4. Fasilitas Pigging&lt;br /&gt;Dalam operasi perminyakan, pigging dilakukan secara berkala untuk membersihkan internal pipe dari kotoran kotoran (endapan-endapan lumpur) akibat proses industri ataupun sisa pengeboran minyak ataupun air yang terjebak akibat kontur dari tanah yang dapat mengakibatkan korosi, yang mengakibatkan bocornya pipa minyak ataupun gas yang dapat merugikan perusahaan dan mengancam keselamatan orang. Oleh karena itu perlu pipeline harus dibersihkan dengan pigging sehingga setiap ujung-ujung pipelines biasanya ada alat fasilitas yang disebut pig launcher (peluncur pig) dan pig receiver (penerima pig). &lt;br /&gt;&lt;br /&gt;Di dalam pengoperasian pigging, perlu mengenal alat-alat monitor untuk keselamatan yang terdapat di peluncur dan penerima pig dalam penggunaannya perlu diperhatikan tekanan &amp; temperatur kerja sebelum ataupun sesudah meluncurkan pig.&lt;br /&gt;&lt;br /&gt;Yang perlu diperhatikan dalam pekerjaan pigging adalah sebagai berikut :&lt;br /&gt;1. Gambar skematik dari masing-masing pig launcher (peluncur) dan pig receiver (penerima).&lt;br /&gt;2. Prosedur kerja pada saat akan memasukan pig (baik pig biasa ataupun pig khusus), bagian-bagian (valve/line) mana yang akan dibuka dan ditutup.&lt;br /&gt;3. Ijin kerja yang harus disetujui dan diketahui oleh pertugas setempat yang berwenang baik pada saat peluncuran maupun penerimaan.&lt;br /&gt;4. Tempat kerja yang memadai untuk melakukan pekerjaan.&lt;br /&gt;5. Usahakan melakukan pigging pada waktu siang hari.&lt;br /&gt;6. Adakan safety briefing/talk sebelum melakukan pekerjaan dan dihadiri oleh semua pihak terkait dan diketahui oleh petugas setempat yang berwenang dan bertanggung jawab ditempat tersebut.&lt;br /&gt;7. Lakukan pemeriksaan ulang sebelum melakukan pigging kondisi alat-alat monitor tekanan dan temperatur, pig signal serta jenis pintu-pintu penutup-peluncur dan penerima dapt bekerja dengan baik.&lt;br /&gt;8. Mengetahui media (gas/minyak/kondesat/air) di pipeline tersebut serta kandungan-kandungannya seperti H2S, CO2 dll.&lt;br /&gt;9. Gunakan radio komunikasi yang akurat  antara tempat peluncur maupun penerima.&lt;br /&gt;10. Mempunyai hirarki satu komando yang bertanggung jawab.&lt;br /&gt;&lt;br /&gt;Pekerjaan pigging dilakukan termasuk dalam tugas pemeliharaan dan mempunyai jadwal tertentu menurut kebutuhannya tergantung terhadap media terkandung. &lt;br /&gt;&lt;br /&gt;5. Perlengkapan Keselamatan pada Proses Produksi&lt;br /&gt;Beberapa perlengkapan keselamatan yang mendasar pada proses produksi minyak akan dibicarakan berikut ini.&lt;br /&gt;&lt;br /&gt;a. Subsurface Equipment&lt;br /&gt; Pemakaian dari Subsurface Equipment adalah suatu peralatan yang dipasang ditubing dibawah permukaan tanah untuk pencegahan aliran yang bertekanan abnormal apabila terjadi keadaan yang tidak dapat dikontrol baik disurface atau di subsurface sehingga dapat menyebabkan Blow-Out atau semburan liar di well head (kepala sumur).pada lapangan minyak ataupun gas yang mempunyai tekanan reservoir yang tinggi.(lihat gambar 3).&lt;br /&gt;&lt;br /&gt;Peralatan ini biasa disebut sebagai Sub-Surface Safety Valves (SSSVs) dan dipasang di dalam sumur minyak/gas didalam tubing produksi yang dalamnya bervariasi tergantung dari karakteristiknya reservoir.&lt;br /&gt;&lt;br /&gt;SSSVs merupakan suatu alat pengaman yang sangat penting didalam suimur minyak dan gas bumi dan ini dipasang atas persyaratan oleh API RP14A, allat ini ditest dalam peride tertentu sesuai kebijakan perusahaan masing-masing.   &lt;br /&gt;&lt;br /&gt;Ada 2 jenis SSSV yang dikenal yaitu :&lt;br /&gt;&lt;br /&gt;1. Jenis flapper valves (lihat gambar 4).&lt;br /&gt;2. Jenis Ball valves (lihat gambar 5).&lt;br /&gt;&lt;br /&gt;Dari masing-masing jenis tersebut diatas mempunyai suatu keunggulan dan kelemahan sendiri-sendiri. Alat SSSV ini dipakai didalam sumur minyak atau gas bumi adalah sebagai alat pengaman demi keselamatan kerja baik untuk orang atau aset perusahaan dan biasanya peggunaan ini didalam standard API RP-14A.&lt;br /&gt;&lt;br /&gt;Untuk SSSV biasanya sudah diset oleh perusahaan jasa perminyakan dan kontraktor untuk masalah ini biasanya sudah mengetahui cara-cara menanganinya. &lt;br /&gt;&lt;br /&gt;b. Emergency Shutdown System (ESD)&lt;br /&gt;&lt;br /&gt;Kegunaan atau pemakaiannya :&lt;br /&gt;Sistim ESD selalu dipasang dan berguna apabila proses fasilitas produksi baik proses didarat ataupun anjungan minyak dilepas pantai mengalami situasi darurat &lt;br /&gt;ESD sistem adalah sebuah sistem untuk mematikan sistem produksi hydrokarbon dengan menarik tombol, dengan ditariknya tombol ESD bekerja secara tekanan angin (pneumatic) dan menutup semua valve yang dioperasikan dan mematikan seluruh sistem produksi baik itu anjungan lepas pantai ataupun proses fasilitas produksi didarat termasuk pemutusan listrik ke Reda Pump untuk beberapa field yang menggunakan sumersible pump, ESD sistem harus dapat dioperasikan setiap saat baik dalam kondisi pengeboran/ service ulang sumur atau dalam kondisi normal produksi.&lt;br /&gt;&lt;br /&gt;Biasanya ESD dipasang di lokasi yang strategis seperti :&lt;br /&gt;1. Landing Boat.&lt;br /&gt;2. Helipad.&lt;br /&gt;3. Proses fasilitis.&lt;br /&gt;4. Dekat dengan driller box pada waktu pengerjaan sumur minyak/gas.&lt;br /&gt;5. Setiap dek di tangga jalan keluar.&lt;br /&gt;6.  Dekat jalan keluar dari ruang tempat tinggal (living quarter).&lt;br /&gt;7.  Lokasi lain yang dirasakan perlu dipasang ESD.&lt;br /&gt;&lt;br /&gt;Setelah mengenal bagian pokok dalam peralatan produksi dan bagaimana cara kerjanya, hal pokok yang perlu diperhatikan adalah peralatan tersebut harus memperhatikan jadwal inspeksi dan pemeliharaan tahunan agar senantisa dapat bekerja setiap saat apabila terjadi keadaan darurat (emergency situation)  yang tiba-tiba. Sistem safety produiksi dirancang dan ditempatkan pada bagian-bagian yang telah diperhitungkan dan kemungkinan terjadinya tekanan &amp; temperatur yang berlebihan dan ini harus tetap diperhatikan oleh seluruh pekerja apabila bekerja dilapangan minyak &amp; gas bumi.&lt;br /&gt;&lt;br /&gt;Di dalam pemakaian alat safety produksi ada juga yang menggunakan alarm sebagai signal atau tanda-tanda apabila terjadi kondisi tidak normal, baik itu overpressure atau underpressure. &lt;br /&gt;Peralatan sistem safety produksi dirancang untuk mencegah terjadinya kecelakaan yang dapat merugikan baik perusahaan maupun manusia. &lt;br /&gt;Dan perlu mengenal safety faktor yang sudah ditentukan didalam buku-buku referensi seperti API RP-14A,B,C,D, ANSI, NACE, AWS dsb.&lt;br /&gt;&lt;br /&gt;Sebagai seorang pekerja dibidang perminyakan perlu mengutamakan faktor-faktor keselamatan kerja dan selalu sigap dan waspada akan kondisi kerja kita dan apabila menemukan kondisi abnormal dilingkungan kerjanya agar segera menghubungi petugas setempat yang berwenang untuk mengambil tindakan pencegahan sedini mungkin agar tidak merugikan pekerja dan perusahaan tersebut. &lt;br /&gt;&lt;br /&gt;Mintalah petunjuk kerja kepada petugas setempat yang berwenang, apabila bekerja ditempat yang belum terbiasa, jangan ambil tindakan sendiri yang belum dimengerti benar. &lt;br /&gt;&lt;br /&gt;Gunakan alat keselamatan diri atau pelindung perorangan apabila bekerja disetiap tempat di fasilitas produksi  &lt;br /&gt;&lt;br /&gt;Kegunaan kursus dasar ini sangat penting untuk setiap pekerja dilapangan minyak dan gas bumi serta mempunyai dasar pengertian yang sama terhadap alat-alat produksi dan bagian-bagiannya yang terlibat dalam aktivitasnya dan ini perlu dilihat dari segi keselamatan kerja yang mempengaruhi reputasi dimana ia ditempatkan bekerja. DURING TIMES OF HEAVY WORKLOAD,&lt;br /&gt;LEADERS SHOULD MANAGE THE SITUATION WHILE OTHERS&lt;br /&gt;MANAGE THE ACTUAL TASKS.&lt;br /&gt;Why? Research has shown that such a strategy reduces multitasking and&lt;br /&gt;improves decision-making and vigilance. Leaders who fail to delegate during high&lt;br /&gt;workload risk increased errors and accidents due to the burden of trying to&lt;br /&gt;simultaneously managing the tasks, the team, and the environment. Evidence of&lt;br /&gt;this phenomenon comes from a handy statistic in commercial aviation. The&lt;br /&gt;National Transportation Safety Board found a disproportionately high percentage&lt;br /&gt;of aviation accidents (over 80%) occur when the captain is the pilot doing the&lt;br /&gt;actual flying and the first officer is the pilot "not flying." These captains are&lt;br /&gt;overloaded as they try to accomplish flight tasks and command duties at the&lt;br /&gt;same time.&lt;br /&gt;An example: in our own industry involved a charge nurse failing to delegate. A&lt;br /&gt;patient with a history of an elevated heart rate spontaneously went into a fast&lt;br /&gt;irregular rhythm. The bedside nurse called out for help when the rhythm caused&lt;br /&gt;a significant drop in blood pressure. The charge nurse, three additional bedside&lt;br /&gt;nurses, and the medical resident arrived at the bedside. The resident ordered an&lt;br /&gt;intravenous medication, which the charge nurse went to obtain and then&lt;br /&gt;administered. Rather than delegate the task to the other staff members, which&lt;br /&gt;would allow her to better supervise and document the efforts of the staff, the&lt;br /&gt;charge nurse sent the other three nurses away stating she could handle the&lt;br /&gt;situation. The patient did not respond to the medication and the physician&lt;br /&gt;ordered that the patient be cardioverted (an electrical shock to trigger a normal&lt;br /&gt;rhythm) and that a continuous medication be given intravenously. The charge&lt;br /&gt;nurse left to mix the medication and get the defibrillator machine. The charge&lt;br /&gt;nurse returned to administer the electric shock with the physician present. The&lt;br /&gt;patient continued to deteriorate and needed additional treatment that was&lt;br /&gt;delayed because the charge nurse did not delegate tasks to the other team&lt;br /&gt;members. The patient suffered mild heart ischemia, and a low oxygen level that&lt;br /&gt;required mechanical ventilation to assist breathing, and resulted in a prolonged&lt;br /&gt;ICU stay.&lt;br /&gt;Reference: Sexton, J.B. Mistakes often occur because care team members lack clarity about&lt;br /&gt;exactly what they are to do, or when they are to do it. Research has shown that&lt;br /&gt;asking a question early (rather than asking the same question later) not only&lt;br /&gt;reduces the risk of failure or injury, but also saves time and reduces the stress&lt;br /&gt;that comes with ambiguity and uncertainty.&lt;br /&gt;An example: An intern joined rounds late. At the end of rounds, the senior house&lt;br /&gt;officer noted that it was time to remove the first of the two chest tubes in an&lt;br /&gt;elderly patient. While everyone present nodded in agreement, the task was given&lt;br /&gt;to the late intern, who did not know which tube was placed first and did not feel&lt;br /&gt;comfortable asking after having nodded knowingly moments before. He&lt;br /&gt;incorrectly assumed the patient was the one that he participated in rounds on.&lt;br /&gt;Later, the intern went to carry out the task, and incorrectly removed what looked&lt;br /&gt;like the older tube. Had he asked questions when first assigned the task, he&lt;br /&gt;would not have removed the wrong chest tube from the wrong patient. This&lt;br /&gt;caused the patient respiratory distress requiring placement of another chest tube.&lt;br /&gt;Reference: Sexton, J.B. (2004). Leadership behaviors are most important in situations that are complex,&lt;br /&gt;unusual, or high workload. In fact, research has shown that the presence of&lt;br /&gt;leadership behaviors during routine or standardized situations is associated with&lt;br /&gt;poor outcomes. During routine situations, a leader who is present but does not&lt;br /&gt;have to actively engage a technical task should take the opportunity to observe&lt;br /&gt;“Leaders Toward Safety ”&lt;br /&gt;Summarized &amp; compiled by : Satrio Pratomo&lt;br /&gt;the strengths and weaknesses of the team as this knowledge is critical during&lt;br /&gt;less routine situations, when the ability to predict and understand the behaviors&lt;br /&gt;of others is essential.&lt;br /&gt;An example: A patient with a very unstable airway and a recent tracheotomy&lt;br /&gt;(surgical incision into the trachea through the neck) suddenly suffered an&lt;br /&gt;obstructed airway and within a few seconds was experiencing life threatening&lt;br /&gt;hypoxia (low blood oxygen levels) and hypotension. The resident and fellow were&lt;br /&gt;not sure how to manage the patient, and had been unsuccessfully paging the&lt;br /&gt;ICU attending for the past several minutes. A surgical attending unfamiliar with&lt;br /&gt;the patient happened upon the scene in the ICU and immediately assumed&lt;br /&gt;charge of the patients care. She attempted dilation of the airway unsuccessfully.&lt;br /&gt;She requested a scalpel and a replacement trachea, replaced the artificial airway&lt;br /&gt;and stabilized the patient while the resident and fellow stepped aside to assist&lt;br /&gt;her. Within a few minutes, the patient stabilized and the attending left the care of&lt;br /&gt;the patient in the hands of the fellow.&lt;br /&gt;Reference: Kerr, S. and Jermier, J. M. (1978). We often lose time and information in multidisciplinary settings because&lt;br /&gt;transparency of actions between and within disciplines requires discussions with&lt;br /&gt;colleagues from other disciplines in various physical locations with differing&lt;br /&gt;degrees of accessibility. Setting public daily goals (click here for an example) for&lt;br /&gt;a given patient using a goals sheet attached to a clipboard at each bedside&lt;br /&gt;enhances the transparency of actions and intentions between and within&lt;br /&gt;disciplines for "this patient, in this bed, today." Daily goals are associated with a&lt;br /&gt;50% decrease in length of stay, by providing a public record of thought&lt;br /&gt;processes and decision making by the multidisciplinary team.&lt;br /&gt;An example: During rounds the attending physician and fellow agreed that it was&lt;br /&gt;best to gently diurese the patient in order to move toward extubation. The patient&lt;br /&gt;“Leaders Toward Safety ”&lt;br /&gt;Summarized &amp; compiled by : Satrio Pratomo&lt;br /&gt;had a history of coronary artery disease patient was the one that he part and&lt;br /&gt;chronic renal insufficiency (the patient was greater than 10 kg above preoperative&lt;br /&gt;weight). By the end of the day, the patient was stable and progressing&lt;br /&gt;according to plan. After evening rounds the attending went home and the senior&lt;br /&gt;resident wrote to increase the lasix gtt to 3 mg an hour. This deviation in the plan&lt;br /&gt;of care resulted in the patient becoming hypotensive requiring fluid boluses and&lt;br /&gt;and a neosynepherine gtt to stabilize the patient's blood pressure. Sambil minum “Teh Poci” ala&lt;br /&gt;warteg, obrolan terus mengalir, mengenai keluarga, dan pekerjaan masing&lt;br /&gt;masing, yang akhirnya disalah satu sesi obrolan tsb. dia bercerita tentang&lt;br /&gt;masalah berat yang dia hadapi di tempat kerjanya saat ini.&lt;br /&gt;“Saya pusing karena harus menangani ”kematian” karena kecelakaan&lt;br /&gt;kerja (fatality),” kata teman saya yang bernama BADROEN. Yang membuat&lt;br /&gt;saya turut pusing adalah perusahaan BADROEN ini telah mendapat ”Zero&lt;br /&gt;Accident-Safety Award” berkali-kali. Ini setidaknya mungkin secara sistem,&lt;br /&gt;perusahaan ini telah bagus dan lengkap dari ISO ke OHSAS 18001.&lt;br /&gt;”Saya lagi tidak beruntung karena usaha saya dalam meningkatkan&lt;br /&gt;disiplin di tempat kerja selama tiga tahun telah gagal,” kata BADROEN&lt;br /&gt;yang lagi bingung sambil menggaruk bagian kepalanya yang licin . Apabila&lt;br /&gt;kita lihat data kecelakaan di tempat kerja selama tiga tahun berturut-turut,&lt;br /&gt;hasilnya cukup bagus yaitu kecelakaan kerja yang terjadi adalah first aid.&lt;br /&gt;Saya sebagai teman baik dan concern dengan masalah sefety merasa ikut&lt;br /&gt;prihatin dan menganalisa kenapa kecelakaan kerja yang sangat parah&lt;br /&gt;(fatality) masih bisa terjadi pada perusaaan yang mempunyai sistem yang&lt;br /&gt;sudah bagus tersebut. Hasil diskusi dan analisa lebih lanjut menjelaskan&lt;br /&gt;bawa Safety belum membudaya secara baik.&lt;br /&gt;&lt;br /&gt;Empat Budaya Safety.&lt;br /&gt;&lt;br /&gt;Budaya Reactive:&lt;br /&gt;Didalam budaya ini, safety didelegasikan kepada Safety Department. Kalau&lt;br /&gt;ada kecelakaan semua orang sibuk dan kesalahan selalu ditujukan kepada&lt;br /&gt;safety, Management Commitment hampir tidak ada. Untuk mencegah&lt;br /&gt;kecelakaan, Petugas Safety berpatroli selama 24 jam, mengawasi terus&lt;br /&gt;menerus. Pada budaya ini tingkat kecelakaan kerja masih sulit dikendalikan&lt;br /&gt;atau “ Zero is Unrealistic”. Kecelakaan kerja kapan saja akan datang kalau&lt;br /&gt;pengawasan mengendur. Sebagai perumpamaan polisi tertidur, kecelakaan&lt;br /&gt;meningkat.&lt;br /&gt;&lt;br /&gt;Budaya Dependent:&lt;br /&gt;Didalam budaya ini, safety sudah menjadi kondisi untuk bekerja. Peringatan&lt;br /&gt;keras akan diberikan bila ada pelanggaran safety. Management Commitment&lt;br /&gt;mulai ada. Safety Procedure diperbaiki terus menerus. Safety Training wajib&lt;br /&gt;diberikan kepada semua orang yang bekerja. Pada budaya ini tingkat&lt;br /&gt;kecelakaan kerja telah membaik tetapi mencapai Kesinambungan&lt;br /&gt;(Sustainable) Zero Accident masih sulit atau ”Zero is Difficult”. Kecelakaan&lt;br /&gt;kapan saja akan datang kalau komitment menurun.&lt;br /&gt;&lt;br /&gt;Budaya Independent:&lt;br /&gt;Didalam budaya ini, safety sudah menjadi personal komitment dari semua&lt;br /&gt;orang. Manajemen telah turun langsung ke lapangan untuk melakukan&lt;br /&gt;management audit. Safety menjadi tanggungjawab lini operasi. Safety&lt;br /&gt;standard telah diinternalisasi didalam semua proses bisnis. Pengetahuan&lt;br /&gt;safety telah menjadi kebutuhan dan kompetensi manajemen. Pada budaya ini&lt;br /&gt;tingkat kecelakaan kerja telah mencapai Zero (Kadang-kadang) atau ”Zero is&lt;br /&gt;by chance”. Pada budaya ini bagian Safety Department sudah berapda posisi&lt;br /&gt;yang cukup stabil dan aman.&lt;br /&gt;&lt;br /&gt;Budaya Interdependent:&lt;br /&gt;Budaya safety inilah yang menjadi tujuan bagi semua organisasi. Safety&lt;br /&gt;sudah menjadi kebanggaan bagi semua orang di dalam organisasi karena&lt;br /&gt;semua orang menceritakan safety. Terjadi saling menjaga dan mengingatkan.&lt;br /&gt;Semua pegawai mempunyai otoritas mencegah kecelakaan dengan menegur&lt;br /&gt;bahkan memberhentikan proses bila proses ini membahayakan karyawan dan&lt;br /&gt;perusaan. Pada budaya ini kecelakaan kerja telah mencapai Zero yang&lt;br /&gt;berkesinambungan atau ”Zero by Choice”. pada budaya ini Safety&lt;br /&gt;Department telah menjadi Centre of Excellence.&lt;br /&gt;Bagaimana memperbaiki budaya?&lt;br /&gt;BADROEN menjadi tertarik ketika saya menceritakan empat budaya safety&lt;br /&gt;ini. Dia menanyakan bagaimana caranya (How to implement it?)&lt;br /&gt;” Pertemukan saya pada Top Manajemen Perusahaan dan biarkan saya&lt;br /&gt;menceritakan keempat budaya ini”, jawab saya. Jawaban ini sepertinya tidak&lt;br /&gt;begitu berkenan bagi BADROEN, ”Kenapa Top Manajemen, apakah dengan&lt;br /&gt;saya tidak cukup?”&lt;br /&gt;Saya menjawab, perubahan budaya dimulai dari kepemimpinan. Hanya&lt;br /&gt;Pemimpin yang bisa membawa perubahan karena perubahan ini mencakup&lt;br /&gt;seluruh orang yang berada di organisasi. Perubahan ini harus dimulai dari&lt;br /&gt;perubahan sikap dan proses kepemimpinan. Jangan berharap terjadi&lt;br /&gt;perubahan yang drastis kalau sikap pemimpin masih sama (status quo).&lt;br /&gt;Selain pemimpin, apalagi?” Tanya BADROEN ini.&lt;br /&gt;”Anda”, jawab saya.&lt;br /&gt;”Loh kok saya?” BADROEN tertegun. Untuk merubah sesuatu,perubahan&lt;br /&gt;juga dimulai dari diri kita sendiri. Banyak organisasi yang tidak mau berubah&lt;br /&gt;karena orangya tidak mau berubah.&lt;br /&gt;Perubahan yang diharapkan dari orang safety adalah perubahan paradigma&lt;br /&gt;bahwa safety adalah tanggungjawab lini operasi bukan tanggungjawab safety&lt;br /&gt;department. Ada ketakutan dari orang safety bila paradigma ini diterapkan&lt;br /&gt;akan mengurangi pekerjaan dan tanggungjawab mereka. Walaupun&lt;br /&gt;sebenarnya ini bukanlah hal yang harus ditakuti karena dalam pepatah bisnis,&lt;br /&gt;”Semakin kita takut kehilangan, maka kita semakin dekat dengan kehilangan.”&lt;br /&gt;&lt;br /&gt;Perubahan paradigma ini memang akan merubah tanggunjawab orang safety&lt;br /&gt;yang lebih banyak melakukan pengawasan secara reguler. Sehingga mereka&lt;br /&gt;mempunyai waktu yang lebih untuk berpikir dan menganalaisa untuk&lt;br /&gt;melakukan perubahan. Mereka akan mempunyai waktu untuk berdiskusi&lt;br /&gt;dengan orang lapangan dan konsultan, dan melihat kondisi kerja saat ini yang&lt;br /&gt;akhirnya mereka mampu membuat road map perubahan. Road Map&lt;br /&gt;perubahan yang terintegrasi dengan perubahan organisasi menjdikan safety&lt;br /&gt;department ”Agent of Change” yang lambat laun meningkatkan tingkat&lt;br /&gt;kompetensi orang safety. Peningkatan kompetensi beserta peningkatakan&lt;br /&gt;pengaruh akan merubah peranan safety dari ”Inspektur” menjadi ”Advisor”.&lt;br /&gt;Akhir kata....&lt;br /&gt;Setelah berdiskusi cukup panjang ini, BADROEN menjadi mengerti dan&lt;br /&gt;memahami, makna safety sesungguhnya dan kita membuat janji untuk&lt;br /&gt;mendiskusikan rencana pertemuan dengan direksi di tempat ia bekerja.&lt;br /&gt;BADROEN merasa ”Lega” setelah mendengar cerita saya ini. Make sure machine is properly grounded.&lt;br /&gt;2. Never permit “live” parts of the electric welder to touch bare skin or wet clothing.&lt;br /&gt;3. Do not cool electrode holders by emersion in water.&lt;br /&gt;4. Turn off power supply when welder is not in use.&lt;br /&gt;5. Do not stand on wet areas while welding.&lt;br /&gt;6. Wear leather gloves.&lt;br /&gt;7. Make sure cable are covered and in good condition.&lt;br /&gt;8. Make certain that electrode holders are properly insulated.&lt;br /&gt;&lt;br /&gt;B. BURNS&lt;br /&gt;1. Protect eyes and face from flying particles of slag by use of safety glasses or face shield.&lt;br /&gt;2. Wear adequate protective clothing.&lt;br /&gt;3. Always wear leather gloves.&lt;br /&gt;4. Wear high top shoes.&lt;br /&gt;5. Keep collar, shirt pockets, etc buttoned.&lt;br /&gt;6. Do not touch the electrode or metal where welding has taken place.&lt;br /&gt;7. Handle hot metal with pliers or tongs.&lt;br /&gt;8. Keep electrode stubs properly disposed of.&lt;br /&gt;C. Radiant Energy (utlra violet rays, etc.)&lt;br /&gt;1. Use a welding helmet with the correct shade lens in good condition.&lt;br /&gt;2. Wear suitable clothing—do not leave bare skin exposed to the rays of the arc.&lt;br /&gt;3. Do not strike the arc without covering the face and eyes.  Give warning to others before striking the arc.&lt;br /&gt;4. Avoid looking directly at the arc where others are welding without proper eye protection.&lt;br /&gt;D. GASSES AND FUMES&lt;br /&gt;1. Work only in well-ventilated areas.&lt;br /&gt;2. Use great care when working on metals covered with lead or zinc.&lt;br /&gt;3. If working in a confined area use respirator or other approved breathing devices.&lt;br /&gt;E.  Combustible Materials&lt;br /&gt;1. Keep shop clean in areas where welding is to be done.&lt;br /&gt;2. Do not weld near combustible materials of any kind.&lt;br /&gt;3. Never weld on covered containers which may have held combustible materials without first taking adequate safety precautions.  For example, fill them with water, steam clean or fill with an inert gas.&lt;br /&gt;F.  Fire Protection&lt;br /&gt;1. Be familiar with location and types of fire extinguishers.&lt;br /&gt;2. Report any unsafe conditions that might start a fire.&lt;br /&gt;3. Do not weld near inflammable materials.&lt;br /&gt;4. Do not weld on containers that have held inflammable materials.&lt;br /&gt;5. Definition&lt;br /&gt;1. An accident is an undesired event that does result in an injury, occupational illness, property damage, spill, production loss, fire, etc.&lt;br /&gt;&lt;br /&gt;2. An incident is an undesired event that does, or has the potential to, result in an injury, occupational illness, property / equipment damage, spill, production loss, fire, etc.&lt;br /&gt;&lt;br /&gt;3. A Critical / Sensitive Incident  is an incident having significant impact. This includes Fatality, Serious Injury, Major Fire, Blow-out, Major Gas Leak, Explosions, Natural Disaster, and 15 Barrels and over Oil Spill to Major Rivers or Malacca Strait.&lt;br /&gt;&lt;br /&gt;4. A fatality is a loss of life due to an accident.&lt;br /&gt;&lt;br /&gt;5. A lost time injury is a fatality or lost work day case. The number of LTIs is the sum of fatalities and lost work day cases.&lt;br /&gt;&lt;br /&gt;6. Lost Work Day Case is any work related injury or illness other than a fatal injury which results in a person being unfit for work on any day after the day of occurrence of the injury or illness. “Any day” includes rest days, weekend days, leave days, public holidays or days after ceasing employment.&lt;br /&gt;&lt;br /&gt;7. Restricted Work Day Case (RWDC) is any work related injury or illness other than a fatality or lost work day case which result in a person being unfit for full performance of the regular job on any day after the occupational injury or illness.&lt;br /&gt;&lt;br /&gt;8. A medical treatment is an injury where a doctor or medical facility is required to treat the injury. Employee is able to work on the following day.&lt;br /&gt;&lt;br /&gt;9. A first aid is an injury accident where no doctor or medical facility is required for treatment (minor cuts, scrapes, bruises, etc.) which must be recorded in the First Aid Record Book at the location.&lt;br /&gt;&lt;br /&gt;10. A near miss is an undesired event that has the potential to result in an injury, occupational illness, property/ equipment damage, spill, production loss, fire, etc. (The addition of one outside factor present at the scene of the near miss would have resulted in an accident).&lt;br /&gt;&lt;br /&gt;11. A motor vehicle accident (MVA) is any incidents involving a motorized vehicle designed for transporting overland people and goods when it is in motion resulting in harm to people and/or property damage of any amount.&lt;br /&gt;&lt;br /&gt;12. A preventable motor vehicle accident (PMVA) is any MVA in which a driver failed to do everything reasonably possible to prevent the MVA.&lt;br /&gt;&lt;br /&gt;13. jargon-jargon iklan obat batuk?&lt;br /&gt;&lt;br /&gt;Beberapa komentar paling sering tentang safety dan lingkungan;&lt;br /&gt;&lt;br /&gt;- kalo gak ada aturannya gak usah dipaksain....&lt;br /&gt;- Gak ada yang liat....buang aje diselokan limbahnya...&lt;br /&gt;- Udahlah lo kerja buat pabrik atau buat pemerintah sih??&lt;br /&gt;- Gak usah terlalu idealis lah...pelanggaran kecil mah nggak apa-apa..&lt;br /&gt;- Gak kok gak ada kecelakaan...(padahal kepalanya di perban)&lt;br /&gt;- Udah dik, yang penting kagak ada yang benjol, gak usah bikin laporan lah....&lt;br /&gt;- Woi woi....ada orang safety tuh, buruan  pake helm...pake sarung tangan.....kacamata jangan lupa...&lt;br /&gt;&lt;br /&gt;Waduh ternyata komentar-komentar kayak gini sering sekali di terima sama teman-teman saya, yang (mungkin) kultur safety di perusahaannya belum sebaik teman-teman yang tidak pernah dikomentari seperti itu...&lt;br /&gt;Safety (kadang) cuma jadi nama...lambang-lambang....slogan-slogan....sebatas retorika saja....sedih kalo memang demikian adanya.&lt;br /&gt;&lt;br /&gt;Mungkin penyebabnya karena komunikasi yang kurang mengena, atau cara sosialisasi yang terkesan arogan, tidak punya kekuatan secara instansi, atau malah (yang paling buruk) sebenarnya tidak ada komitmen yang total top management terhadap safety...&lt;br /&gt;&lt;br /&gt;Jeleknya bencana dan kecelakaan kadang menjadi barometer pelaksanaan K3...&lt;br /&gt;hey bukankah kita ada untuk mencegah??kita ada untuk memprediksi??kita ada untuk mempersiapkan kemungkinan terburuk yang bisa terjadi....kalo nunggu kecelakaan dulu baru beraksi...reaktif adalah jawaban yang salah untuk membawa kultur safety di tempat kerja....ujung-ujungnya kita bisa di panggil "polisi India" datang kalo ada kecelakaan, kalo gak ada apa-apa gak pernah keliatan.....jelek betul kedengaranya&lt;br /&gt;&lt;br /&gt;Maka tantangan itu di serahkan kepada kita, seberapa mampu kita membuat However, it is recognized that complete prevention of recurrence by a single intervention is not always possible. Thus, RCA is often considered to be an iterative process, and is frequently viewed as a tool of &lt;a href="http://en.wikipedia.org/wiki/Continuous_improvement" title="Continuous improvement"&gt;&lt;span style="text-decoration: none; color: rgb(0, 0, 0);"&gt;continuous improvement&lt;/span&gt;&lt;/a&gt;. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/p&gt;  &lt;p class="MsoNormal" style="line-height: normal;"&gt;&lt;span style=""&gt;Root cause analysis is not a single, sharply defined methodology; there are many different tools, processes, and philosophies of RCA in existence. &lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt;  &lt;p class="MsoNormal" style="line-height: normal;"&gt;&lt;b&gt;&lt;span style=""&gt;General principles of root cause analysis&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/b&gt;&lt;/p&gt; This bio-predator scheme has saved Rp. 1.3 billion annually in one site only.&lt;br /&gt;Other technology and management applications in eco-efficiency are; the utilization of solvent tanks in&lt;br /&gt;vehicle sales operation branches yielding up to Rp. 450,000 per month, the utilization of ash from&lt;br /&gt;incinerators (hazardous waste) for brick material thus reducing disposal material to landfill(P.T. Pantja&lt;br /&gt;Motor), and the application of re-usable coolants for machinery(P.T. Denso Indonesia). Other applications&lt;br /&gt;of EHS practices enforce key EHS principles: Refine, Reduce, Re-use, Recycle, Recover and Retrieveto-&lt;br /&gt;Energy. Implementation is regularly monitored and motivated through training programs (EHS Officer&lt;br /&gt;Development and Eco-Efficiency Training Programs). Evaluation for training participants includes&lt;br /&gt;practical projects presented by participants within three months of in-class training.
